How do you know where the line is between a game being too difficult, which spoils the overall effect, or you just being crap at it?

Honestly, I think that depends entirely on you. I could use Conra 4 as an example, which I have. I know it's a hard game, because I can barely get past the first level, but at the same time, I know I'm not that great at it because my brother managed to beat it well enough on easy.
What game are you debating this difficult question with, anyway?

if it feels fair, or if you got fucked by the camera/some dude popping up out of nowhere/a quick-time event/the game not giving you a save or checkpoint or even health refill before a boss/whatever.

It's more that Thief 3's difficulty is so high at times, even right from the start, that it becomes frustrating rather than challenging, and reward is overcome by relief. It's not that the game's unfair. It's just plain too difficult.

I agree with bluberry, really. There are some games that are too difficult for me, but I don't consider them unfair or cheap and thus the level of difficulty isn't something that I would knock, merely note when reviewing. If a game is flat out ridiculous in how it handles the difficulty--and includes many of the elements bluberry mentioned--then I would start taking off points accordingly and be very specific when noting what's wrong with that aspect of the game (since even the cheapest of games will have their fans who might write--or think--well, he's just rubbish at a perfectly good game). Also prepare yourself for those who will write and say "My grandma can beat this blindfolded, you wuss!" because you just know you'll hear crap like that. ;-)

Contra 4's easy mode is essentially baby mode. It can be tough on normal, though, especially with the dual screen gameplay.
